Mechanism
Adsorption here is physical rather than chemical. A gram of activated charcoal carries on the order of a thousand square metres of pore surface, and non-polar organic molecules stick to it by van der Waals forces. It works only on what is still in the gut lumen, and it does not meaningfully bind alcohol, iron, lithium, cyanide, strong acids or alkalis, which is why the hangover and detox marketing does not follow from the pharmacology. It will also adsorb any other drug taken at the same time, including oral contraceptives.

Subjective profileweighing the evidence above
Almost every jar sold for wellness is sold on a mechanism that does not apply to it. Adsorption stops at the gut wall, so nothing already in the blood is touched, and alcohol, iron, lithium and the strong acids and alkalis barely bind at all; the detox and hangover claims are an emergency department indication borrowed for marketing. The genuine hazard of casual use is the opposite of what buyers expect, because the same indiscriminate binding strips whatever else is in the stomach, oral contraceptives included. In poisoning it is a real intervention with a narrow time window, which makes it a hospital decision rather than a cupboard staple.

Notes and cautions
- The serious harm from activated charcoal is pulmonary aspiration, which can cause charcoal pneumonitis, ARDS and death; it must not be given to anyone with a depressed or unprotected airway, and it should not be given if the airway cannot be secured first.
- Do not give it for corrosive ingestions, where it obscures endoscopy and offers nothing, or for hydrocarbons, where aspiration risk is already high.
- Charcoal also adsorbs oral medicines and enteral nutrition indiscriminately, so it can render regular prescriptions ineffective, and repeat dosing with sorbitol-containing preparations has caused dehydration and electrolyte disturbance, particularly in children.
- The consumer trend of taking activated charcoal daily as a 'detox' or whitening product has no supporting evidence and carries a real risk of blunting the absorption of prescription drugs, including oral contraceptives.
