Mechanism
The clinically limiting component is lipase, because fat maldigestion is what produces steatorrhoea and weight loss, and lipase is the enzyme most easily destroyed by gastric acid. Preparations are therefore enterically coated to survive the stomach and release at duodenal pH. Dosing is counted in lipase units against fat intake rather than by tablet, and Mezym Forte is a low-strength product compared with the enteric microsphere preparations used for true pancreatic insufficiency and cystic fibrosis.

Notes and cautions
- There is no boxed warning.
- The signature serious risk is fibrosing colonopathy, a stricturing colopathy reported in children with cystic fibrosis at high lipase doses, which is why dosing is capped at roughly 10,000 lipase units/kg/day; all manufacturers were required by FDA to study this risk [3].
- Common adverse effects are gastrointestinal (abdominal pain, nausea, altered stool) and allergic skin reactions; hyperuricosuria and hyperuricaemia can occur with high doses.
- Because the product is porcine-derived, it carries a theoretical viral-transmission concern and is unacceptable to some patients on religious or dietary grounds, and it is contraindicated in known pork protein hypersensitivity.
