Generally well tolerated; mild GI upset and occasional headache are the main sides. Because of antiplatelet activity, caution is warranted with anticoagulants or around surgery. It's a widely used, low-risk bioflavonoid overall.

Rutin has almost no human interaction data, and the honest summary is that what exists is preclinical.
The best characterized mechanism is antithrombotic. Rutin inhibits extracellular protein disulfide isomerase, an enzyme platelets and endothelium require for thrombus formation, and that inhibition blocked thrombosis in animal models before the finding was carried into a small clinical program. On mechanism alone, adding rutin to warfarin, a direct oral anticoagulant or an antiplatelet drug is a plausible route to additive bleeding, though no controlled study has measured it in people.
The second signal is pharmacokinetic. In rats, rutin given with oral paclitaxel raised the drug's peak concentration and total exposure substantially, an effect attributed to inhibition of P-glycoprotein and CYP3A4 in the gut wall; rutin also inhibits CYP3A4 in vitro. Whether flavonoid concentrations reachable from a supplement do the same in humans is untested, and rutin's poor oral bioavailability argues against extrapolating freely. Both signals are best read as theoretical rather than documented.
