Metamizole is a pyrazolone painkiller and fever reducer that is unavailable in much of the English speaking world because of a rare but potentially fatal collapse of the white cell count.

Mechanism
Its active metabolites inhibit cyclooxygenase centrally and, unusually for a non-opioid, also act on the endocannabinoid and descending inhibitory systems, which is why it relieves colic when NSAIDs do not. Peripheral anti-inflammatory action is weak.

Subjective profileweighing the evidence above
Few drugs split national regulators this cleanly: an excellent analgesic for colic where NSAIDs fail, and a drug the United States removed in 1977 because it can silently destroy the white cell count. The agranulocytosis is idiosyncratic, so no amount is safe and a good previous course predicts nothing about the next one; fever or a sore throat during treatment is the presentation and it needs a blood count that day rather than a wait. Germany, Spain, Poland, Brazil and Russia have judged that risk acceptable, and anyone taking it should at least know they have joined a side in that argument.
