Mechanism
As an essential amino acid, lysine is incorporated into new proteins during translation and supplies the reactive side chains that the body modifies for structural and metabolic purposes. Certain lysine residues are hydroxylated and then joined together by lysyl oxidase to form the cross-links that stabilize collagen and elastin fibers, and lysine also feeds the biosynthesis of carnitine, which transports fatty acids for energy production. The proposed antiviral action rests on the balance between lysine and arginine: because herpes simplex replication favors arginine, raising lysine intake is thought to compete with and limit arginine availability, an effect seen in laboratory settings. Lysine is generally very safe at typical doses; high doses can cause GI upset like cramps or diarrhea. Very high long-term intake is not well studied and could theoretically stress kidneys in people with existing kidney disease. It is otherwise low risk.

Lysine has a thin interaction profile, and saying so plainly is more useful than padding it out. Note also that ibuprofen lysine, a neonatal injectable, is a different substance; its interaction profile is ibuprofen's and does not transfer to lysine as a supplement.
The one interaction with a clear mechanism is with arginine. The two share the cationic amino acid transport system in the gut and at the renal tubule, so each competitively reduces uptake of the other; taken together in quantity they blunt one another. That matters mostly when arginine is being taken for a specific purpose of its own.
Lysine also increases intestinal calcium absorption and reduces urinary calcium loss, so in theory it adds to high-dose calcium and vitamin D. Reports of clinical hypercalcemia from that combination are absent. A suggested potentiation of aminoglycoside nephrotoxicity is likewise theoretical, resting on shared renal handling rather than on any case series.
