The most important long-term risk is retinal damage that depends on dose and duration, which is why doses are kept at or below 5 mg per kg of actual body weight and eyes are screened after about five years of use. Common milder effects include nausea, stomach upset, rash, headache, and skin or hair pigment changes. Rare but serious problems include heart muscle disease, QT prolongation, and low blood counts. It can add to the QT effect of other drugs, raise digoxin levels, and it is generally continued in pregnancy because it is considered safe there.

Hydroxychloroquine prolongs the QT interval, and the interactions that can actually kill are cardiac. Given with azithromycin, fluoroquinolones, amiodarone, sotalol, ondansetron, methadone or antipsychotics such as haloperidol, the effect is additive and the risk of torsades de pointes rises, particularly where hypokalemia or bradycardia is already present.

Hydroxychloroquine lowers blood glucose in its own right and has caused severe hypoglycemia without any antidiabetic drug present; added to insulin or a sulfonylurea the effect compounds.
Two further points are worth knowing. Antacids raise gastric pH and reduce hydroxychloroquine absorption, which is why the two are conventionally taken about four hours apart. And concurrent tamoxifen raises the risk of hydroxychloroquine retinopathy roughly fivefold, an odds ratio of 4.6 in the largest long-term screening cohort.
