Mechanism
Framycetin is neomycin B, an aminoglycoside that binds the bacterial 30S ribosomal subunit and causes misreading of mRNA; gramicidin punches cation permeable channels through Gram positive membranes; dexamethasone suppresses the inflammatory response that produces the swelling and pain. Two antibiotics with different targets cover a wider range than either alone.

One question decides whether this is safe to put in an ear: whether the eardrum is intact. An aminoglycoside that reaches the middle ear through a perforation can damage hearing permanently, and the discharging painful ear that most tempts somebody to reach for these drops is the ear most likely to be perforated. That is not a judgement anybody can make by looking in a mirror, which is the entire argument for finding someone with an otoscope first. The eye indication carries the separate steroid problem, because a herpetic or fungal keratitis worsens under dexamethasone while the redness improves, and that is the most dangerous kind of feedback.
