Erythromycin is the original macrolide antibiotic, now used mainly in penicillin allergy, for atypical respiratory pathogens, and at low dose as a gut prokinetic rather than as an antibiotic at all.

Mechanism
It binds the 50S ribosomal subunit and blocks bacterial protein synthesis. Separately it is a motilin receptor , which is why low doses are used to speed gastric emptying in gastroparesis, and it is a strong CYP3A4 inhibitor with a QT prolonging effect of its own.
