Rare cases of hepatitis and liver failure have been reported with black cohosh, so overlap with other hepatotoxic drugs compounds an already uncertain risk. Randomized trials of the standardized isopropanolic extract have not shown liver injury, which leaves the signal unexplained rather than dismissed.
