Mechanism
It is a selective and D3 with essentially no affinity for serotonergic, , histaminergic or cholinergic sites, which is why it settles agitation without the broad receptor load of a phenothiazine. That same selectivity makes prolactin elevation prominent.

For an agitated older patient or an alcohol withdrawal state, a dopamine blocker without the antihistamine and anticholinergic load of the old phenothiazines is a sensible tool, and that clean receptor profile is the entire argument for tiapride. Selectivity is not the same as gentleness, though: prolactin rises sharply precisely because nothing else is competing, the QT interval lengthens, and the drug can cause the tardive dyskinesia it is often prescribed to suppress.
