Doxazosin is a long acting alpha-1 adrenoceptor blocker used for benign prostatic hyperplasia and, much less often now, for hypertension.

Mechanism
Blocking alpha-1 receptors relaxes vascular smooth muscle, lowering peripheral resistance, and relaxes the smooth muscle of the prostate and bladder neck, easing urinary flow. It is not subtype selective, unlike tamsulosin, so the urinary benefit always arrives with the blood pressure effect rather than instead of it.

Subjective profileweighing the evidence above
Doxazosin lost its place in hypertension when the ALLHAT arm was stopped early for excess heart failure, and there is no good argument for choosing it for blood pressure now. For prostate symptoms it still works, but the absence of subtype selectivity means the pressure drop arrives whether it is wanted or not, which in an older man is a fall risk rather than a footnote; the first dose and every increase are when syncope happens, and evening timing is the usual way around it. One thing belongs in a conversation with an eye surgeon before cataract surgery, since alpha blockers cause intraoperative floppy iris syndrome and the operation goes better when the surgeon knows in advance.
