Mechanism
Meldonium inhibits gamma-butyrobetaine hydroxylase, lowering carnitine synthesis so cells shift from fatty acid oxidation towards glucose oxidation, which yields more ATP per unit of oxygen. Emoxypine succinate is a 3-hydroxypyridine radical scavenger that also delivers succinate straight into the respiratory chain. The manufacturer asserts the two are synergistic; that specific claim rests on its own trials.
