Mechanism
Uridine triacetate is uridine with its three hydroxyl groups acetylated, which greatly improves oral absorption (plain uridine is largely broken down on first pass); esterases then release free uridine. Uridine is a pyrimidine nucleoside that feeds the CDP- (Kennedy) pathway used to build neuronal membranes (phosphatidylcholine), supports membrane synthesis, and can lift and signaling; it also acts at P2Y pyrimidinergic receptors. Clinically the triacetate is a rescue agent for fluorouracil/capecitabine overdose and for hereditary orotic aciduria.
