Overview
Enoxaparin is a low-molecular-weight heparin (LMWH), made by chemically breaking unfractionated heparin, derived from pig intestinal mucosa, into shorter polysaccharide chains. Enoxaparin is used to prevent venous thromboembolism (clots in the deep veins and lungs) in hospitalized and surgical patients, to treat established deep-vein thrombosis and pulmonary embolism, and in the management of acute coronary syndromes, including unstable angina and heart attack. In acutely ill medical patients confined to bed, the MEDENOX trial showed that enoxaparin markedly reduced the risk of venous thromboembolism compared with placebo [3].
In acute coronary syndromes, enoxaparin was compared head-to-head with unfractionated heparin. Enoxaparin is prescription only and given by injection. The main risk is bleeding, from minor bruising at the injection site to serious internal hemorrhage. It can lower platelets, and although less often than unfractionated heparin it can still trigger heparin-induced thrombocytopenia, a paradoxical clotting reaction. Long-term use may weaken bone. It carries a boxed warning about spinal or epidural hematoma when used around neuraxial anesthesia or spinal puncture, which can cause lasting paralysis. Combining it with antiplatelet drugs, NSAIDs, or other anticoagulants raises bleeding risk. Because it is cleared by the kidneys, doses must be lowered in significant kidney impairment. Protamine sulfate only partially reverses its effect.

Heparins also suppress aldosterone, so pairing enoxaparin with ACE inhibitors, ARBs or potassium-sparing diuretics can push potassium up, especially alongside kidney impairment or diabetes.

History
Enoxaparin was among the first of the low-molecular-weight heparins, a class created by chemically depolymerizing standard heparin into shorter, more uniform fragments to obtain a more predictable anticoagulant.
