Levoamphetamine is the other half of Adderall. Mixed amphetamine salts are three parts dextroamphetamine to one part levoamphetamine, and this is the part almost nobody talks about, despite it being the main pharmacological difference between Adderall and plain dextroamphetamine. Overview
Levoamphetamine, the l or R enantiomer of amphetamine, exists in practice as a constituent rather than a product. Mixed amphetamine salts, sold as Adderall and its relatives, contain dextroamphetamine and levoamphetamine in a fixed three-to-one ratio; racemic amphetamine products contain them equally. Pure dextroamphetamine products contain none of it, and that absence is the entire pharmacological difference between the two families.
The enantiomers differ substantially in potency and in balance. Animal work comparing them directly found clearly differential activity, with the dextro form far more potent on the dopamine-mediated behaviours that characterise amphetamine, while the levo form retains more of its activity on noradrenaline [1]. The practical consequence is a different ratio of central to peripheral effect: levoamphetamine contributes proportionally more to cardiovascular and sympathetic effects and less to the focused, reinforcing central action.
It is also cleared more slowly than the dextro form, which is part of why mixed salts have a longer tail than an equivalent dose of dextroamphetamine alone. Patients who describe Adderall as harsher, longer or more physically noticeable than Dexedrine are describing an experience whose likeliest pharmacological explanation is this component.

Since the attentional and reinforcing effects of amphetamine track most closely, and since the cardiovascular effects track noradrenaline, the levo delivers a profile weighted away from the first and toward the second.
Its slower clearance relative to the dextro form contributes the long tail characteristic of mixed salts. Where the cardiovascular side is the limiting problem with mixed salts, switching to pure dextroamphetamine is a recognised clinical move, and this component is the reason it can help. That decision belongs with a prescriber.
History
Amphetamine was first synthesized in 1887 and reintroduced medically in the 1930s as the racemate, an equal mixture of both enantiomers, sold as Benzedrine. The distinction between the two forms was therefore not initially a clinical choice at all; the racemate simply was the drug.
Dextroamphetamine was separated and marketed on its own from the 1930s and 1940s onward as the more potent central stimulant, and that established the practical division that persists: pure dextro products on one side, and mixtures on the other. Mixed amphetamine salts, with their fixed three-to-one ratio, arrived much later and reintroduced levoamphetamine into mainstream ADHD treatment as a deliberate formulation choice rather than a historical accident.
The experimental characterisation of how the enantiomers differ belongs mostly to the 1970s and 1980s, when direct behavioural comparisons established that the dextro form dominates the dopamine-mediated effects while the levo form retains relatively more noradrenergic activity [1]. That literature is old, and it has not been substantially revisited with modern methods, which is part of why the clinical significance of the ratio remains more argued than settled.
Reputation
Levoamphetamine has almost no independent reputation, which is itself notable given how many people take it daily without knowing the name. It appears in discussion almost entirely as an explanation rather than a subject: the answer to why Adderall and Dexedrine feel different despite containing the same principal drug.
That explanation is widely repeated and is reasonably well founded. The enantiomers do differ substantially in their balance of dopaminergic to noradrenergic activity [1], and it follows that a product containing a quarter levoamphetamine will present differently from one containing none.
Where the discussion overreaches is in precision. The claim that levoamphetamine is responsible for a specific proportion of side effects, or that removing it reliably fixes cardiovascular intolerance, goes beyond the evidence; the enantiomer comparison literature is decades old and was not designed to attribute clinical adverse effects. The defensible version is narrower: the ratio is a real pharmacological difference, and trying pure dextroamphetamine when mixed salts are cardiovascularly intolerable is a reasonable clinical step for a reason rather than a superstition.
