Clorazepate is a long acting prodrug benzodiazepine: stomach acid converts it into nordazepam, also called desmethyldiazepam, the active molecule that carries nearly all of its pharmacology [2]. Double blind trials from the 1970s and 1980s support it as an anxiolytic comparable to diazepam and buspirone [3] [4], and it has a long clinical record in alcohol withdrawal and as an adjunct in refractory epilepsy [6] [8]. Mechanism
Clorazepate is a , not an active agent in its own right. In the acidic environment of the stomach it decarboxylates rapidly to nordazepam (N-desmethyldiazepam); in mice given the drug intravenously, unchanged clorazepate disappeared from plasma within the first hour while anticonvulsant activity tracked nordazepam concentrations [2]. Nordazepam is a positive modulator at the benzodiazepine site of the -A receptor, amplifying GABA mediated chloride currents and producing anxiolytic, sedative, anticonvulsant and muscle relaxant effects. The design smooths absorption: after a single 40 mg oral dose of a nordazepam precursor, peak serum nordazepam concentrations averaged 115 ng/ml at 8.6 hours, with a mean elimination half life of 61 hours [1]. This long lived sustains anxiolysis around the clock with once daily or bedtime dosing, which is also why residual effects can persist into the next day.

The name clorazepate refers to the acid; clinical products use the dipotassium salt.
