Mechanism
7-Hydroxymitragynine is an indole alkaloid that acts as a potent at the mu-opioid receptor, the target responsible for opioid analgesia, euphoria and sedation. It is considerably more potent at the mu receptor than the parent alkaloid mitragynine, and much of kratom's opioid character comes from 7-hydroxymitragynine formed in the body. As a mu-opioid agonist it is reversible by the naloxone.

Concentrated 7-hydroxymitragynine products are far stronger than natural kratom leaf and carry real opioid risks, including sedation, respiratory depression at high doses, dependence and withdrawal. It is dangerous combined with other depressants such as benzodiazepines and alcohol, and with other opioids.
