Minoxidil + Finasteride is a combination therapy widely used for male androgenetic alopecia that pairs two agents with complementary actions: minoxidil, a topical vasodilator that stimulates the hair follicle, and finasteride, a 5-alpha-reductase inhibitor that lowers levels of the androgen dihydrotestosterone (DHT). Together they address both the follicle and the hormonal driver of pattern hair loss, and clinical studies generally find the combination more effective than either drug alone. The two are used together as oral finasteride with topical minoxidil, or in combined topical formulations.
Epristeride is a 5-alpha-reductase inhibitor developed by SmithKline Beecham, taken to phase 3 for prostate enlargement and never approved in the West. It is chemically unlike finasteride: a 3-androstene carboxylic acid rather than an azasteroid, and an uncompetitive inhibitor that mimics the enzyme's transition state [1]. ⚠️ It is now sold for hair loss, an indication its own pharmacology argues against, since scalp 5-alpha-reductase is mostly type 1 and epristeride is a weak type 1 inhibitor [4].
